B-vitamin supplementation lowers homocysteine reliably but RCT evidence for CV/dementia outcome benefit is mixed (VITATOPS, VISP negative for stroke recurrence; some AD biomarker signals positive). Still worth measuring and correcting due to cheap safe intervention, even if causality debated. MTHFR C677T/A1298C polymorphism testing often paired with homocysteine measurement.
