What this is
Alternative to TRT for men who want to preserve fertility. FDA declined to approve enclomiphene citrate (Androxal) in 2015 citing inconsistent data. Compounded enclomiphene widely prescribed by telehealth men's-health clinics. Hohl 2025 systematic review/meta-analysis (Arch Endocrinol Metab) confirmed SERM therapy significantly increases TT (+274 ng/dL vs placebo) LH and FSH; comparable to T gel and superior to hCG monotherapy.
Mechanism
Selective estrogen receptor modulator; blocks hypothalamic estrogen receptors, disinhibiting GnRH → LH/FSH → endogenous testosterone production; enclomiphene is the trans isomer (cleaner profile than mixed clomiphene)
Dose & route
Enclomiphene 12.5-25 mg/day PO; clomiphene 25-50 mg every other day
