What this is
Vastly overused in TRT practice. Most men on standard TRT doses don't need AI; chronic AI at high doses causes iatrogenic low estradiol symptoms often misattributed to other causes. Only intervene if gyno, edema, or severe mood issues develop AND estradiol is clearly elevated.
Mechanism
Selective non-steroidal aromatase inhibitor; blocks conversion of androgens to estradiol; approved for ER+ breast cancer; off-label for estrogen management during TRT or AAS cycles
Dose & route
0.25-0.5 mg 2x/week for TRT users (cautious dosing); breast cancer 1 mg/day
