What the evidence says

Clomiphene has been studied in men where raising LH/FSH and preserving spermatogenesis are outcomes, in contrast with exogenous testosterone's suppressive HPG-axis effects. DailyMed labeling lists visual symptoms including blurred vision and scotomata and calls for discontinuation and evaluation if they occur. Hohl 2025 systematic review/meta-analysis found SERM therapy increased total testosterone by about 274 ng/dL vs placebo and raised LH/FSH. The mixed-isomer drug has more clinical history than enclomiphene but also more estrogen-receptor activity outside the hypothalamus. A second 2026 SR/MA (11 studies, n=1,512; Eur J Clin Pharmacol) found no significant difference in serum testosterone between clomiphene and testosterone gel, but TRT produced better libido scores (ADAM 1-5, p=0.009) -- shows a fertility-preserving treatment context with different sexual-function outcomes from TRT.
