Bottom line

Read Off Label grades Enclomiphene citrate as B- (6.3/10) based on moderate evidence, med benefit magnitude, and a low-med-risk safety profile.

Cleaner pharmacology than mixed clomiphene because it lacks zuclomiphene, the longer-lived estrogenic isomer.

Studied, labeled, or reported-use context: 12. — Investigational / compounded; not individualized guidance.

What the evidence says

Cleaner pharmacology than mixed clomiphene because it lacks zuclomiphene, the longer-lived estrogenic isomer. Phase 2/3 studies showed testosterone restoration while maintaining sperm counts, unlike testosterone gel. FDA did not approve Androxal after the 2015 review cycle, so US access is compounded or research-market rather than an approved product. Hohl 2025 grouped clomiphene and enclomiphene SERMs and found higher TT, LH, and FSH versus placebo; That finding does not establish identical risk.

Mechanism

Trans isomer of clomiphene; estrogen receptor antagonist at hypothalamus/pituitary; raises GnRH → LH/FSH → endogenous testosterone while preserving spermatogenesis in secondary hypogonadism

Studied, labeled, or reported dose & route

12.5-25 mg/day PO in trials and common compounded protocols

This records doses and routes described in studies, approved labeling, clinical practice, or documented use. It is not individualized guidance and does not establish safety; context changes with indication, formulation, health history, monitoring, and other medicines.

Common questions

What does the evidence show about Enclomiphene citrate's effects?
Read Off Label rates the evidence for Enclomiphene citrate as Moderate and the benefit magnitude as med, producing an overall grade of B- (6.3/10). Cleaner pharmacology than mixed clomiphene because it lacks zuclomiphene, the longer-lived estrogenic isomer.
What safety findings are reported for Enclomiphene citrate?
Enclomiphene citrate has a low-med risk profile in the database. Low-Med (appears cleaner than mixed clomiphene in trials; limited long-term and post-market safety; compounded-product quality risk) Legal status: Investigational / compounded (Androxal was not FDA-approved; no FDA-approved US product).
What doses or routes are reported for Enclomiphene citrate?
12.5-25 mg/day PO in trials and common compounded protocols This is context from studies, approved labeling, clinical practice, or documented use; it is not individualized guidance and does not establish safety.
How does Enclomiphene citrate work?
