~800 trials published, mostly from Japan. Modest but consistent effects on oxidative stress, inflammation markers. Cost and logistics of proper H2 delivery are significant. Hype exceeds evidence but basic science is legitimate.
Mechanism
H2 selectively neutralizes hydroxyl radicals and peroxynitrite (most damaging ROS) without affecting signaling ROS; small molecule with high tissue penetration
