What the evidence says
Field has matured 2023-2025 with multiple new SRs and MAs. Cuenca 2024 SR (Int J Mol Sci, "Extra Healthy or a Hoax?") catalogued 25 studies — preliminary signal across CV, metabolic, and exercise endpoints but explicitly flags methodological heterogeneity AND a conflict-of-interest pattern (many trials are sponsored or co-authored by hydrogen-product companies). Yang 2024 SR/MA (8 RCTs n=357 metabolic disorders) — TG and total cholesterol modestly reduced; LDL non-significant. Korotkov 2024 (Antioxidants 13:145) — 3-month electrolyzed HRW RCT in MetS/pre-MetS improved glucose; HbA1c; cholesterol; and redox markers. HYDRAPPET 2025 (PMC12300559) — 8-week HRW in obese adults reduced cravings; improved sleep; lowered LDL; raised plasma GLP-1. Elite-athlete crossover RCTs (Botek 2024 fin swimmers; 2024 handball/skeleton trial) confirm muscle recovery and reduced soreness; H2 is NOT on WADA 2024 prohibited list. Respiratory: Frontiers Immunol 2024 SR/MA — anti-inflammatory effect of high-concentration H2 inhalation in lung disease (asthma; COPD; ALI/ARDS). Cancer adjuvant: 2023 PMC10152878 SR — quality-of-life and chemo-tolerance signal but no survival data; 2024 Springer J Cancer Res Clin Oncol review reinforces adjuvant framing. Delivery matters: inhalation delivers ~4000× the H2 of one 500 mL water bottle and is the route most respiratory/cancer-adjunct trials use; oral water is cheapest but lowest-dose. Bottom line: real basic-science mechanism (Ohsawa 2007 Nat Med); modestly positive but heterogeneous human evidence; persistent COI concerns; better than placebo on several metabolic or recovery endpoints; direct longevity-outcome evidence is absent.
Mechanism
H2 selectively neutralizes hydroxyl radicals and peroxynitrite (the most damaging ROS) without affecting signaling ROS like H2O2 and NO; small molecule with high tissue/membrane/CSF penetration; modulates NLRP3 inflammasome and Nrf2 antioxidant pathway
Studied, labeled, or reported dose & route
Hydrogen-rich water 0.5-1.6 ppm at 1-2 L/day; OR H2 tablets (modern Mg-based tablets transiently reach 5-12 ppm); OR inhalation 1-4% H2 gas 1-2 hr/day (highest absorbed dose by ~3 orders of magnitude vs. water but cost-prohibitive)
This records doses and routes described in studies, approved labeling, clinical practice, or documented use. It is not individualized guidance and does not establish safety; context changes with indication, formulation, health history, monitoring, and other medicines.

Common questions
- What does the evidence show about Hydrogen water / molecular hydrogen (H2)'s effects?
- Read Off Label rates the evidence for Hydrogen water / molecular hydrogen (H2) as Moderate and the benefit magnitude as low-med, producing an overall grade of B (6.8/10). Field has matured 2023-2025 with multiple new SRs and MAs.
- What safety findings are reported for Hydrogen water / molecular hydrogen (H2)?
- Hydrogen water / molecular hydrogen (H2) has a low risk profile in the database. Low (no safety signals across 80+ human trials; not on WADA 2024 prohibited list) Legal status: OTC (tablets, water generators, inhalation devices).
- What doses or routes are reported for Hydrogen water / molecular hydrogen (H2)?
- Hydrogen-rich water 0.5-1.6 ppm at 1-2 L/day; OR H2 tablets (modern Mg-based tablets transiently reach 5-12 ppm); OR inhalation 1-4% H2 gas 1-2 hr/day (highest absorbed dose by ~3 orders of magnitude vs. water but cost-prohibitive) This is context from studies, approved labeling, clinical practice, or documented use; it is not individualized guidance and does not establish safety.
- How does Hydrogen water / molecular hydrogen (H2) work?
- H2 selectively neutralizes hydroxyl radicals and peroxynitrite (the most damaging ROS) without affecting signaling ROS like H2O2 and NO; small molecule with high tissue/membrane/CSF penetration; modulates NLRP3 inflammasome and Nrf2 antioxidant pathway
